The office is responsible for the civil registration program in the local government unit, pursuant to the Civil Registry Law, the Civil Code, and other pertinent laws, rules and regulations issued to implement them.

  • ISSUANCE OF CERTIFIED COPY OF BIRTH, MARRIAGE AND DEATH CERTIFICATES

THE SERVICE:

Civil Registry documents such as births, marriage and death certificates maybe availed of by securing a certified transcript from the Municipal Registry Office.


  • REGISTRATION OF BIRTH AND MARRIAGE CERTIFICATES

THE SERVICE:

Republic Act No. 3753 mandates that acts, events, legal instruments and court orders/decrees concerning the civil status of persons shall be recorded.  The birth of a child must be registered within thirty (30) days from birth at the Municipal Civil Registry Office.

The Certificate of Marriage of a Civil or Church wedding must be submitted within fifteen (15) days after the solemnization of marriage.  Registration of birth and marriage is free of charge, pursuant to Sec. 6, P.D. No. 651.

Birth Certificate

Registration of live birth or birth certificate is a very important document needed by every citizen. This is usually are requirements for school, employment, marriage, passport and many other things. Its authenticity can only be verified through the municipal civil registrar. If you are born in this municipality, you will have to secure this document from the Office of the Municipal Civil Registrar. They also issue a certified true copy of these documents. The fee is P100.00 for abroad and P25.00 for local and it will be processed upon receipt of the application and payment of the fee.

Procedures for securing a Birth Certificate:

  1. Secure verification slip from the Municipal Civil Register and should be ready to give the following information:
    • complete registered name
    • date of birth
    • place of birth
    • name of parents
    • purpose
  2. Proceed to the Office of the Municipal Treasurer for payment of the fee.
  3. Proceed to the Office of the Municipal Treasurer for payment of the fee.
  4. Present official receipt to the Municipal Civil Registrar along with your application.
  5. Within 30 minutes you can get your birth certificate for the purpose indicated in the application.  

Marriage Certificate

Registration of Marriage Contract or marriage certificate is also a very important document needed by every citizen. Especially those who are married and he or she wanted to secure a passport this is an important document, the fee is P100 for abroad and P25 for local.

Procedures for securing marriage certificate:

  1. Secure verification slip from the Municipal Civil Register and should be ready to give the following information:
    • complete name of contracting parties
    • date and place of marriage
    • purpose
  2. Proceed to the Office of the Municipal Treasurer for payment of the fee
  3. Present official receipt to the Municipal Civil Registrar along with your application.
  4. Within 30 minutes you can get your marriage certificate for the purpose indicated in the application. 

  • REGISTRATION OF DEATH CERTIFICATES

THE SERVICE:

The registration of Death Certificate (DC) with the Municipal Civil Registry Office within the period of thirty (30) days is mandatory.

Death Certificate

Registration of Certificate of Death or Death Certificate is a requirement needed for securing any death claims insurance, death benefits, the fee for burial permit is P20.00 and P25.00 for transfer of cadaver. The fee is P100 for Abroad and P25 for local purposes.

Procedures for securing death certificate:

  1. Secure a verification slip from the Municipal Civil Register and should be ready to give the following information:
    • name of deceased
    • date and place of death
    • purpose
  2. Proceed to the Office of the Municipal Treasurer for payment of the fee.
  3. Present official receipt to the Municipal Civil Registrar along with your application.
  4. Within 30 minutes you can get the death certificate for the purpose indicated in the application. 

  • APPLICATION FOR DELAYED REGISTRATION OF BIRTH, MARRIAGE AND DEATH

THE SERVICE:

Delayed registration of birth, marriage or death must be filed at the Municipal Civil Registry Office following the lapse of the prescribed period of 30 calendar days from birth, marriage or death.


  • APPLICATION FOR MARRIAGE LICENSE

THE SERVICE:

All couples (either one or both residents of Bangued of legal age intending to get married must apply for marriage license at the Municipal Civil Registry Office.  A marriage license is valid in any part of the Philippines for a period of 120 days from the date of issue.


  • CHANGE OF FIRST NAME, NICKNAME AND FOR CORRECTION OF CLERICAL OR TYPOGRAPHICAL ERROR

THE SERVICE:

Republic Act 9048 authorizes the Municipal Civil Registrar to:

  1. Correct clerical or typographical errors in any entry in the Certificate of Live Birth, Marriage and Death.
  2. Change of first name or nickname in the civil registry without need of a  judicial order.  However, any petition to correct error that would subsequently change the nationality, age, status or legitimacy of a person is not allowed and must be filed with the proper court.

Republic Act 10172 authorizes the Municipal Civil Registrar to:

  1. Correct clerical or typographical errors in the Day and Month in the Date of Birth of person appearing in the Civil Register without need of a Judicial Order.  The Date of Birth must not be later than its Date of Registration. However, petitions to correct the Year of Birth and the Date of Birth later than its date of registration must be filed with the proper court.
  2. Correct clerical or typographical error in the Sex/Gender of person appearing in the Civil Register without need of a Judicial Order provided that the document owner has not undergone sex change or sex transplant.

Change of Name and Correction of Clerical Error

Under Ord. No. 4 S-2001 Mandated by RA 9048

The petitioner is a natural person filing the petition for the correction of a clerical or typographical error, or petition for change of first name. He must be of legal age and must have direct and personal interest in the correction of clerical error or typographical error.

How to apply:

  1. Petitioner must file for change of first name in the Officer of the Municipal Civil Registrar
  2. The petition shall not be processed unless the petition will support the required documents.
  3. Upon filing, the petitioner must proceed to the Office of the Municipal Treasurer’s Office for payment. 3,000.00 pesos for change of first name and 1,000.00 pesos for correction of clerical error.
  4. The Municipal of civil registrar must accept the application. In case of change of first name, Notice of Posting for 10 days and be published at least once a week for two consecutive weeks in a newspaper of general circulation. Then the MCR sent all the application at the NSO Manila for the decision. It takes 3 to 4 months for the processing. 

  • LEGITIMATION OF NATURAL CHILD

THE SERVICE:

Legitimation is a remedy by which those who were born out of wedlock to be considered legitimate.  Only children conceived and born outside of wedlock of parents who at the time of conception of the former, were not disqualified by any impediment to marry each other, may be legitimated.  Legitimation of children by subsequent marriage of parents shall be recorded in the civil registry office where the birth was recorded.


  • PREPARATION OF ANNOTATED BIRTH CERTIFICATE OF AN ILLEGITIMATE CHILD TO USE THE FATHER’S SURNAME (For registered child with “Unknown” Father) R.A. 9255

THE SERVICE:

The Municipal Civil Registry Office processes the annotation of the Certificate of Live Birth of an illegitimate child to use the father’s surname.

NOTE: Children born August 4, 1988 to March 18, 2004 ‘NOT COVERED BY AUSF.”


  • REGISTRATION AND ANNOTATION OF COURT DECISIONS AND LEGAL INTRUMENTS

THE SERVICE:

Court decisions concerning the status of a person must be registered in the Civil Registry Officer where the court is functioning when ten (10) days after the court decree/order has become final and executory.

Court decisions that must be registered are the following:

  1. Decree of Adoption
  2. Decree of Nullity of Marriage/Declaration of Nullity of Marriage
  3. Decree of Legal Separation
  4. Court decisions or orders to correct, change, cancel or delete entries in any certificate of birth, marriage or death
  5. Declaration of Presumptive death
  6. Registration or voluntary renunciation of citizenship
  7. Court decisions recognizing or acknowledging natural children or impugning or denying such recognition or acknowledgement
  8. Judicial determination of maternity affiliation
  9. Aliases

Legal instruments that have to be registered:

  1. Affidavit of reappearance
  2. Marriage settlement
  3. Admission of paternity and acknowledgement, legitimation, voluntary emancipation of minor, parental authorization or ratification nor artificial insemination
  4. Acknowledgement
  5. Acquisition of Citizenship
  6. Option to elect Philippine Citizenship
  7. Partition and distribution of properties of spouses and delivery of the children’s legitimate
  8. Waiver of right’s interest of absolute

  • ENDORSEMENT OF REGISTRY RECORDS TO THE CIVIL REGISTRAR GENERAL (PSA)

THE SERVICE:

There are instances when Philippine Statistics Authority does not have available records requested by clients, as a last remedy, clients check the availability of records at the Municipal Civil Registry Office.

If the document is available, the Municipal Civil Registrar submits civil registry documents to the Office of the Civil Registrar General (OCRG).


  • ISSUANCE OF SUPPLEMENTAL REPORT

THE SERVICE:

A supplemental report of birth, death and marriage may be filed to supply information inadvertently omitted when the document was registered.

Every supplemental report shall contain the following items except in the case of Marriage wherein items (f) and (g) are not applicable.

  1. Province
  2. Municipal/Municipal
  3. Registry No.
  4. Information inadvertently omitted in the original registration
  5. Name (of child, deceased or containing parties, as the case may be)
  6. Informant (of the supplemental report)
  7. Prepared by
  8. Received at the Office of the Civil Registrar

  • REGISTRATION OF FOUNDLING/ABONDONED CHILDREN

THE SERVICE:

Foundling is a deserted or abandoned infant or child, without known parents, guardians, or relatives, found by another person; or a child in an orphanage or similar institution without facts of birth and percentage.

Registration of the foundling in the Office of the Municipal Registrar shall be undertaken by the finder or charitable institution within thirty (30) days from the date of finding/commitment of the child.

Any report after the 30 day period shall be considered late, and the concerned party (finder) shall be required to state in a sworn statement the circumstances that caused the late reporting to the civil registrar.

Agencies to Report to After Finding an Abandoned Child

    Immediately after finding a foundling, the finder shall report to the:

  1. Barangay Captain of the place where the foundling was found or to a
  2. Police Station/headquarters, whichever is nearer or a convenient to the finder

The finder must then commit the child to the care of the Social Welfare and Development (DSWD) or to a duly licensed orphanage or charitable institutions, and give his/her copy of the Certificate of Foundling registered with the Municipal Civil Registry Office.
